Dr. Curnew often gives out two copies of requisition forms. One form is good for two blood works. So a total of four available requisitions are given for future visits.
Please provide Dr. Curnew with these details to ensure he can contact you or your family doctor, as needed.
If you have noticed any symptoms or health concerns, take note of them. Futhermore, please bring a list of your questions to all your virtual and in-clinic appointment. Dr. Curnew is more than happy to answer any questions that you may have.
Dr. Curnew encourages all of his patients to measure their blood pressure 3x weekly, and taking note of the average. Do this 3 times per week! Please bring your recordings to all appointments.
Make sure to bring all your medications or medications list to EACH and EVERY appointment. It is important to ensure our records are consistently updated.
Check your medication bottles, or with your local pharmacy, to verify how many medication refills you have left. If it is low, keep note of it and let Dr. Curnew know!
If you were given a bloodwork requisition, Dr. Curnew requests that you please complete it before your virtual or in-clinic appointment.
